We want to ensure the safety and well-being of your child at all times. That's why, under Maryland State law and the Archdiocese of Washington's regulations, we need all the forms below completed, signed, and submitted before the start of the school year.
As we gear up for each new school year, ensuring your child is equipped with any necessary medications is crucial. Please complete the proper form for each medication - this is an essential step!
Please ensure that all medications are kept in their original containers and will only be accepted by our staff if they have visible expiration dates.
Our school nurse may need to contact your healthcare provider to confirm any orders, so accurate and complete information on the forms is essential.
We strongly recommend that you make a copy for your records as well.
Remember that neither prescription nor over-the-counter medications can be given to students without a written order from a healthcare provider.

Due to Maryland state law and the Archdiocese of Washington's regulations, where noted, the documents MUST be signed by a healthcare provider. Please check all health forms for BOTH a parent's or guardian's signature and a healthcare provider's signature.
A parent's signature or a prescription is not sufficient, and medication cannot be administered. If any medicine is dropped off without the proper documentation, it cannot be administered by the school nurses, and it will be sent back home.
Parents are welcome to administer medication themselves during the school day.

As your child starts a new school year or experiences a change in health, schedule their physical immediately.
We want to ensure all our students are healthy and prepared for learning while meeting the immunization requirements of the Archdiocese of Washington and the State of Maryland. The current year's requirements are listed on the State of Maryland's Back to School vaccination requirement information page.
Please complete all five sections of Form 3, and ensure that Part II is completed and signed by your child's Health Provider.

An Allergy Agreement and Action Plan form signed by a healthcare provider is required for all children who need an Epi-Pen or have food allergies.

We understand that obtaining a doctor's order may not always be feasible, so we encourage parents to administer the medication at home whenever possible.
WHO NEEDS IT?: Students with Asthma who need inhalers
HOW MANY?: One per affected student

WHY?: Mandated by Maryland State & Archdiocese of Washington

Is your child in need of an inhaler during school hours? Don't forget to get Form 9 signed by a healthcare provider.
WHO NEEDS IT?: Students with Diabetes
HOW MANY?: One per affected student
